Erlanger offers a pharmacy benefit plan through BlueCross BlueShield of Tennessee (BCBST). The pharmacy is included as a part of the Health Benefit Plan and is administered by BCBST in the CAREMARK® network.   Pharmacy Number: T138ERLA – RX 04

Maintenance Drug List

Home Delivery Mail Service
Medications ordered through CAREMARK® mail service offers the convenience of home delivery and also gives the employee an option of 2 co-pays for a 3 month prescription on any drug covered by the pharmacy plan (does not have to be on the maintenance drug list). For more information you may go to the website www.caremark.com or call 1-877-683-6837. You may also pick up enrollment forms in the Benefit Department of Human Resources.
